HIGH SCHOOL ATHLETICS:
Sports: The forms listed below must be completed at the beginning of each school year by parents and student athletes prior to participating on any CCHS team.

Click on the links below to print out a copy of each of the required forms. Open the Code of Ethics and the Physical Exam forms with the Adobe program. A new set of these forms must be completed before the start of each new school year and returned to the Athletics Office. Forms filled out for one school year cannot carry over to the next school year.

Athletic Fees:
$150 per sport; billed to the student's account. Athletic fees pay for league dues, referees, field rentals, replacement equipment and uniforms as well as trophies/medals, certificates and other expenses related to the annual sports awards night.

Physical Exams Reminder:
All athletes need to take a physical exam before the first game of their season.

ADMISSION FEES FOR HIGH SCHOOL GAMES:
In order to help offset costs of our athletics program and to keep up with rising costs an admission fee will be charged for high school boys' and girls' Home basketball games. Admission fees are:

Adult $4.00
Students with ASB Card (Visiting Team) $2.00
CCHS Students (with ASB Card) and children - Free

Coaches, Asst. Coaches, trainers, players and parents who are staffing the scorers table, the ticket sales table or the snack shack during home games do not have to pay.

Parent Involvement Program (PIP) - PARENTS! Sign Up Now to serve in our Snack Shack during middle and high school home games, as drivers for Away games and as scorekeepers and scoreboard operators (not hard to do at all) and ticket sellers (Home high school games only). Sign up forms are kept in the school office--first come first served basis. Earn those 20 community hours (and more) quickly and support our teams!

PIP Chips for Fall season (girls H.S. volleyball; boys' M.S. football and girls' M.S. basketball) can be picked up at the school office. See Mrs. Mulady.

b>HIGH SCHOOL P.E.
Students are expected to be in the proper P.E. uniform every class session. A 20% point deduction will be made from the daily participation grade for being out of uniform.

Gym locks/lockers are issued to all PE students and to those participating in a sport. Only PE or sports-related items may be stored in the gym locker--no school books, food or drink. Locks must be returned at the end of a sports season or the end of P.E. semester. Lost/damaged locks = $10 charge.

Quarterly Test:
The 2nd Quarter written exam will be given the week of January 12th.

Grading breakdown: 10% test; 15% demonstrated skill level; and 75% participation. P.E. classes are Monday through Friday for one semester.

B = 80 - 89
C = 70 - 79
D = 60 - 69
F = 59 & below

b>MIDDLE SCHOOL ATHLETICS:
Students who wish to play in any sport during the school year must have their parents complete three Medical Consent and Release Forms. All three forms (with original signature) must be returned to the Athletics Director prior to participation on any CCA team.

Winter - Boys Basketball; Girls Volleyball
Spring - Boys Volleyball; Girls Fast-pitch Softball

Athletic Fees:
Billed to the student's account ($125). Athletic fees pay for league dues, referees, field rentals, replacement equipment and uniforms as well as trophies, certificates and other expenses related to the annual sports awards night. There is no charge for attending middle school Home games.

Awards:
Three boys and three girls from each team and one eighth grade boy and girl are selected by the coaches to receive special awards at the Annual Sports Awards Night held in May each year. Click on the link below to review the list of the awards and their explanation.

Girls flag football in the Spring (after spring sports season is completed) This will be a series of after-school classes/practices to learn football basics as well as defensive and offensive skills required to play an official game. At the end of the teaching/practices will be a flag football game between the girls. There will be no charge for this activity.

MIDDLE SCHOOL P.E.
P.E. classes are held every day. Students will be expected to be in the proper P.E. uniform every class session. A 20% point deduction will be made from the daily participation grade for being out of uniform.

Grading breakdown: 10% project, 5% test, 10% skills evaluation, 75% participation. Points (20%) are deducted from the daily participation grade for each day student is not in correct P.E. uniform (light gray T-shirt and dark blue shorts). P.E. classes are Monday through Friday for one semester.

2nd Quarter Project (2008-09):
